Google Forms is a survey administration software included as part of the free, web-based Google Docs Editors suite offered by Google. The service also includes Google Docs, Google Sheets, Google Slides, Google Drawings, Google Sites, and Google Keep. Google Forms is only available as a web application.
To get started, make sure one has a Gmail address. Then follow this link here. One can start from Scratch by selecting Blank or use any of the given templates.
The blank template will look like the above.
To add a new question, click on the plus icon ( + ) at the far right of each question. There are many features one can add to the questions, such as description, images, videos, import questions from other google form questions and so on. Add the questions one prefers.
To publish, click on Send.
The form link, one can also embed the form on their website.
