Microsoft Forms is an online survey creator, part of Office 365. Forms allows users to create surveys and quizzes with automatic marking. The data can be exported to Microsoft Excel.
To get started, make sure one has a Microsoft account. Then follow this link here. One can start from Scratch by selecting Blank or use any of the given templates.

To add a new question, select the plus icon ( + ) and one is provided with various question options. One can even preview and change the theme of the form. Choose the ones one prefers.
Edit the form to one’s liking.
Click on Send to publish.
