Woobox is a social promotion application. It allows you to run sweepstakes, contests, polls, and more across Facebook, Twitter, Google+, YouTube, LinkedIn, and Pinterest.

The campaign page, click on Landing pages and find Form.
Click on Form.
The default form. To remove fields, click on the bin icon and to add fields, click on Add Form Field. To edit the form field, click on the Form Fields particular form field one would love to edit or the click on the input field of the previewed site.
Edit it to one’s liking. Note that one can view it with a mobile view or desktop view. To publish, click the back icon ( < ) on the top left corner.
The back button takes one to the overview of the site. Click on Publish.
Click on Publish Campaign.
The form is published.
